Acute Care is designed for patients that need to see a doctor for a single visit.  Examples would include a periodic check up, prescription refill, acute illness.

The cost of an acute care visit is $150

Continuity Care

Continuity Care is designed for patients needing regular medical attention from their physician. Often, Continuity Care patients are seen on a regular basis to make sure chronic medical conditions are well controlled.
The cost of Continuity Care is $175 per month
